Both points are marked and measured against each other and against the natural shape of your lips — not against a fixed template, because every mouth is a different width and shape.
They are checked with your mouth relaxed and with a natural smile. Points that look perfectly even with your mouth still can look uneven the moment you speak or smile, because lip tissue moves and stretches differently in different people.
You confirm the marks in a mirror before either piercing happens. Take your time here. Once both holes are pierced, adjusting spacing is not simple.

Does it hurt?

Swelling — expect more than a single lip piercing

Two piercings on the lower lip means more total swelling than one, and it is worth planning around properly.

Three to five days of noticeable swelling, sometimes affecting the whole lower lip rather than staying localised to each point.
Eating and speaking are more awkward than with a single piercing, simply because more of the lip is affected at once.
The initial jewellery on both sides is sized slightly long to accommodate this. It will look larger than the final look at first — that is intentional.
Ice, used gently, helps in the first day or two.

The teeth risk — applies twice over

Healing two piercings at once

8 to 12 weeks is realistic, on the longer end for lip piercings, because two healing sites on the same small area of tissue means more overall inflammation and a bit more time for everything to settle.
They do not always heal at exactly the same pace. One side settling slightly ahead of the other is common and not a problem — just be patient with the slower side rather than treating both the same at every stage.
Double the vigilance on aftercare, simply because there are two sites to manage rather than one.

Mouth care during healing

Rinse with alcohol-free antiseptic mouthwash or saline solution after eating, covering both sides thoroughly, for the first few weeks.
Sterile saline spray on both outer piercings, same as any fresh piercing.
Avoid spicy, acidic, very hot or very cold food for the first week — with two piercings, irritation from food affects a larger area of the lip.
No smoking, vaping, alcohol or kissing for at least the first two weeks.
Do not fidget with either piece against your teeth or against each other.
Soft food and careful eating while swelling is present, more so than with a single lip piercing.

What can go wrong

Asymmetry. The main preventable risk, and why the marking stage matters so much before anything is pierced.
Gum recession and enamel wear. Covered above, doubled by having two pieces of jewellery. Preventable with correct flat-back jewellery and sizing.
Uneven healing between the two sides. Common and usually not a concern, just a reason for patience.
Migration on one or both sides. More likely if a bar was too short or too long.
Infection. Increasing pain after the first few days, spreading redness, thick discharge or fever on either side means a doctor, not a piercer.
