There is one honest conversation that has to happen before any lip piercing, and most studios skip it because it is not a fun thing to say to a paying customer: the wrong jewellery, worn long enough, can wear away your gums and, eventually, damage the enamel on the back of your teeth.

This is not a scare story. It is a well-documented, entirely preventable risk, and the fix is simple — the right jewellery, the right size, and knowing what to watch for. Lip piercing types

Labret. Centred, directly below the lower lip. The most common and versatile lip piercing.
Vertical labret. Through the lip vertically rather than straight through, visible on the outer lip surface as two beads. A striking variation.
Monroe. Off-centre above the upper lip, referencing the classic beauty mark placement. One of the most requested lip piercings.
Madonna. The mirror position on the opposite side to a Monroe.
Medusa (philtrum). Centred directly above the upper lip, through the philtrum. Bold and very visible.
Angel bites. Two piercings symmetrically above the upper lip. See our note on multiples below.
Ashley. Through the centre of the lower lip itself, positioned to sit visibly on the lip surface. One of the more demanding lip piercings to heal well.

The teeth and gum risk — read this properly

This is genuinely the most important section on this page, and it applies to every placement that sits on the inside of the lip against your teeth and gums — which is most of them.

What actually happens: the back of the jewellery — the flat disc or ball that sits inside your mouth — rests against your gums and the inner surface of your teeth. Over months and years of constant contact, this can wear away gum tissue (gum recession) and, in more serious cases, chip or wear the enamel on the back of the teeth.

This is preventable, almost entirely through jewellery choice.

Flat-back labret studs, not rings or balls. A flat disc against the gums does far less damage than a rounded ball, which concentrates pressure and rocks against the tooth surface with every movement.
The correct length, checked as swelling goes down. A bar that was sized for initial swelling and never downsized sits too long and presses harder than it should.
Regular dental check-ups. Tell your dentist you have a lip or tongue piercing. They will check for early gum recession or enamel wear, which is far easier to manage caught early.
Avoid playing with the jewellery. Clicking it against your teeth constantly, a very common unconscious habit, accelerates wear significantly.

Does it hurt?

Genuinely — less than most people expect. Lip tissue is soft and the piercing is quick, usually a sharp pinch for a second or two, similar to or slightly less than an earlobe. The philtrum and vertical labret can feel marginally sharper because of the angle and depth involved, but none of these are placements people describe as severe.

Swelling — plan for it

Lip and mouth-area piercings swell significantly more than most placements, because lip tissue is soft and highly vascular.

Expect noticeable swelling for three to five days, sometimes longer.
This is why the initial jewellery is sized slightly long — to allow room for the swelling. Do not panic if it looks too big at first.
Ice, used gently and not directly on the piercing, helps in the first day or two.
Eating and speaking will feel awkward for a few days. Soft food, and take it slow.

Healing — 6 to 12 weeks, longer for some placements

Labret and Monroe: typically 6 to 8 weeks for the outer healing, though internal healing continues longer.
Medusa and Ashley: often on the longer end, 8 to 12 weeks, because of more movement and contact with teeth or food.
Vertical labret: similar timeline, with the added consideration of two visible points.

Do not change the jewellery early, but do have the bar length checked and downsized by the studio once swelling settles — usually around the two to three week mark. This one step does more to protect your teeth and gums than almost anything else on this page.

Mouth care during healing — different from other piercings

Rinse with alcohol-free antiseptic mouthwash or a saline solution after eating, for the first few weeks. This is specific to oral piercings.
Sterile saline spray on the outer piercing, the same as any other fresh piercing.
Avoid spicy, acidic, very hot or very cold food for the first week — it genuinely irritates a fresh piercing more than people expect.
No smoking, vaping, alcohol or kissing for at least the first two weeks. All of them introduce bacteria or irritate healing tissue directly.
Do not play with the jewellery. Clicking it against your teeth is a habit that starts during the boredom of healing and often continues for years.
Soft food and careful eating while swelling is present.

What can go wrong

Gum recession and enamel wear. Covered above — almost entirely preventable with flat-back jewellery and correct sizing.
Swelling that does not settle. If swelling is severe, worsening after day three or accompanied by difficulty swallowing or breathing, see a doctor immediately.
Migration and rejection. Lip tissue can push jewellery out over time, particularly with placements like the Ashley or if a bar was too short.
Infection. Increasing pain after the first few days, spreading redness, thick discharge or fever means a doctor, not a piercer.

Implant-grade titanium, always. Reactive metal in constant contact with mouth tissue and saliva is a bad combination.

A piercer who does not raise the teeth and gum conversation, or who reaches for a ball-end ring by default, is setting you up for a problem years down the line rather than at the counter.

Can a lip piercing damage my teeth?

Yes, if it is fitted with the wrong jewellery or the wrong length and left that way for years — the back of the jewellery resting against gums and teeth can cause gum recession and enamel wear over time. It is almost entirely preventable with a flat-back stud instead of a ball or ring, correct sizing checked as swelling goes down, and not clicking the jewellery against your teeth.

Which lip piercing hurts least?

Can I kiss or drink alcohol after getting a lip piercing?

Not for at least the first two weeks. Both introduce bacteria to fresh piercing sites and can irritate healing tissue directly. Smoking and vaping should also be avoided for the same reason during that window.

What is the difference between a Monroe and a Medusa piercing?

A Monroe sits off-centre above the upper lip, in the position of a classic beauty mark. A Medusa sits centred directly above the upper lip through the philtrum. Should I get a ring or a stud for my lip piercing?

A flat-back labret stud, especially for the initial piercing and for any placement touching the inside of your mouth. A rounded ring or ball concentrates more pressure against gums and teeth and rocks with movement, which is exactly what causes long-term dental wear. Save rings for after full healing, if at all.
